Community Medicine Clinic
We work to improve the overall wellness of Grand Rapids' homeless and low-income residents, regardless of resources or ability to pay. This is perhaps our most important service as the health of any community depends on all of its residents' health.
The Spectrum Health Medical Group Community Medicine Clinic offers full-service primary care with after hours on-call nurse triage for urgent needs. We have an integrated behavioral health model to serve populations seeking to access mental health services in the community. Our goal is to help develop patient-centered plans of care. We also provide support and education with a nurse case manager who specializes in the management of patients with chronic disease.
We support individuals and family members who are struggling with mental health disorders, including substance use disorder and addiction, and help connect patients with substance use disorder services in the community. We provide LGBT-aware care with on-site transgender healthcare. Additionally, we work closely with homeless populations and newly-arrived refugees.
We provide a full range of medical and mental health services to treat the whole person.
